For Medicare purposes, the "look back" period is five (5) years--that is, they will look at, and possibly reverse or take back, any transactions (e.g. gifts, transfers, below-market value sales, etc.) which occured up to five years ago. For private medical costs, there is no set look back period, though if a transfer, etc. is made after a large private bill was incurred, the provider who is not getting paid could try to set the transfer aside as a fraud on creditors.
